Our intake process is designed to be simple and automatic:
Receive Forms Automatically
Immediately after booking, you'll receive an email with all required forms
The email will be from Calendly with subject "Belay PLLC - Appointment Confirmation"
Complete Forms Online
Click the secure link in your email and complete forms on any device
No printing or scanning required
Ready for Appointment
Once forms are submitted, you're all set for your telehealth session
Video link will be sent 24 hours before your appointment
Important Deadline
All forms must be completed 48 hours before your appointment.
Incomplete forms may result in appointment rescheduling.

If forms are not completed 48 hours before your appointment:
- We will send a reminder email 3 days before your appointment
- If still incomplete 48 hours before, we may need to reschedule
- This ensures we have time to review your information before your session
